Home from Hospital Service
Oaklea Trust
The Home from Hospital service can prepare the home prior to discharge, for e.g. ensuring basic necessities are in and that the heating is on. It can assist with a wide range of tasks such as light cleaning, preparing light snacks, dog walking, shopping and prescription collection. The Home from Hospital programme cannot provide personal care, moving & handling or assist/prompt medication. Please refer to Adult Social Care in this instance.

General Advice and Support

Advice on minor adaptations to the home (stair rails, bed raisers, bathroom support handles, etc.)
Information about local services, clubs and social activities to reduce isolation.
Advice and guidance around personal budgets and self funding support, enabling people to purchase their own support services.
Advice around maintaining health and wellbeing and assistive technology.

Weekend Access Service
Mind in Furness
Mix of advice and information, triage and signposting for all older and vulnerable adults.
Social contact point and activity.
Community and volunteer development.
Sessions are 10am – 2pm every Saturday at School Street, Barrow. Place to come to chat over tea and coffee, signposting and advice also available. Open door policy (no appointments needed). Support worker or project worker always available.

Aids and Adaptations
British Red Cross 
Guidance on placing and fitting of aids and adaptations linking closely with hospital discharge and equipment loans service using Trusted Assessor model.

Hearing aid maintenance & support.
Advice and guidance on using hearing aids.
Support to reduce isolation and improve confidence to engage socially.
