NHS Building the Right Support: Transition from Hospital Fund

A single grant to support up to twelve adults with a mild learning disability and/or Autism to support referrals from NHS Cumbria, Northumberland Tyne & Wear NHS Foundation Trust to transition from hospital back to the community to achieve one or more of the following outcomes:
• reduction in length of stay
• reduction in revolving door admissions
• increased engagement with statutory sector where required, to help manage the service user’s health and wellbeing needs including attendance at health appointments.
• increased engagement/attendance at community activities including education and employment.
• support the individual to develop strategies and techniques to promote self-management.
• capturing of qualitative data including patient experience and stories which can be used to help evaluate services and inform on further service development.

Who can apply?
Community groups and charitable, not-for-profit organisations who are specialist in supporting users with a mild learning disability and/or autism and who can cover the full North Cumbria ICB footprint.

Expectations of project delivery:
• work with the identified cohort to build and maintain trusting working relationships through the period of transition from inpatient wards to the community
• work will commence initially in inpatient settings to support discharge planning and through to the discharge to community including where appropriate in discharge to assess beds
• work as part of the multi-disciplinary team both in an inpatient setting and community setting to maximise successful discharge planning
• secure active and sustained engagement with statutory and community services, where necessary
• provide a focus on education, community activities and to support of physical and mental wellbeing to maintain a healthy lifestyle and to aid in lapse prevention
• reduce isolation and provide a support system to aid in service users managing their condition

What the Fund will not support in addition to our normal exclusions:
• service users under the age of 18
• those with a risk profile which is not suitable for 1:1 support
• people with primary diagnosis or substance misuse
• providing a diagnosis, medical services or prescribing medication

How much can you apply for?
The grant will be up to £40,000 for one year. Partnership applications are welcome.

Submission and consideration dates
Closing date for applications is Friday, 14 March 2025. Applicants will be notified by 1 April 2025.
