Grants to increase the uptake of Annual Health Checks for individuals aged 14+ with a learning disability and/or Autism living in the former districts of Allerdale, Carlisle and Eden.
Desired outcomes:
• improved health outcomes of individuals with a learning disability over the age of 14
• to help meet the national target of more than 75% Annual Health Checks for people with a learning disability. This equates to at least 150 additional health checks.
• improved knowledge of Annual Health Checks and Health Action Plans, and their importance
• improvement in health outcomes from earlier diagnosis and support to manage their health needs
• increased engagement/attendance at health appointments
• decrease the barriers to taking up Annual Health Checks by making people aware of the reasonable adjustments that may be required.
• capturing of qualitative data including patient experience and stories which can be used to help evaluate services and inform on further service development.

Who can apply?
Community groups and charitable, not-for-profit organisations who are specialist in supporting users with a mild learning disability and/or autism and who can cover the full North Cumbria ICB footprint.
Expectations of project delivery:
• work with individuals, families, carers, and schools to increase the uptake of annual health checks from 68% to over the national target of 75% in North Cumbria
• provide education to the above to increase the knowledge about the importance of annual health checks
• promote the use of Health Action Plans linked to the Annual Health Check
• identify barriers to people engaging in Annual Health Checks and seek solutions
• secure active and sustained engagement with statutory and community services, where necessary
• look at a sustainable plan to improve the uptake of assessments beyond March 2026
What the Fund will not support in addition to our normal exclusions:
• providing a diagnosis, medical services or prescribing medication
How much can you apply for?
The grant will be up to £23,000 for one year. A single application, multiple smaller applications and/or partnership applications will be welcome.
Submission and consideration dates
Closing date for applications is Friday, 14 March 2025. Applicants will be notified by 1 April 2025.
